Hello nhtsb, my certified pre owned 2012 ford f-150 lariat 3.5l ecoboost 4wd supercrew has had the safety recall nhtsa 19v-075 19s07 6r80 sudden transmissiondownshift powertrain control module programming on march 18 2019 and since that time the wrench symbol has appeared in the speedo cluster twice and sent the truck into limp mode in motion each time in the late afternoon i was able to limp the truck to the dealer, each time it was too late to have a tech connect the truck to the diagnosis machine ! and if the truck is turned off there is no stored trouble codes ! i continue to drive the truck every day with the erratic shifting issues iam in fear of my truck downshifting to 1st gear on the freeway at high speed !!! i have not felt any improvement to powertrain since the recall repair was preformed, and i see no statemeant that the transmission lead frame was replaced ! thanks for your help. best
Tl* the contact owns a 2012 ford f-150. while traveling approximately 20 mph, the vehicle shook and downshifted into first gear. the vehicle was repaired on august 6, 2019 at beach ford (855 jason blvd, myrtle beach, sc 29577) per nhtsa campaign number: 19v075000 (power train). the same dealer was contacted via phone for the recent failure. the manufacturer was notified of the repair failure. the vehicle was not diagnosed or repaired. the approximate failure mileage was 125,000.
Tl* the contact owns a 2012 ford f-150. while driving 75 mph in fog, the contact attempted to pass a tractor trailer. the vehicle hesitated, failed to accelerate, and decelerated to 60 mph. in addition, the check engine warning indicator illuminated, the rpms fluctuated between 2000-4000, and the gear shifted from fourth to sixth gear. the local dealer and manufacturer were not contacted. the failure mileage was 51,000.
The truck has a recall 16n02 open on it. i just purchased the truck and a few years ago it was in an accident at 99,000 miles and the passenger bedside was replaced, but insurance company totalled it so it has a branded title. dealer and ford are refusing to do the repair due to the title status.
While going over speed bumps around 10 mph the truck tends to make a "clunk" sound and the engine revs real high - as if it missed a gear.

Tl* the contact owns a 2012 ford f-150. while driving 75 mph, the vehicle downshifted independently and the engine revved to 4,000 rpms. the contact immediately decreased the speed to 60 mph and the vehicle independently shifted back into drive. the vehicle was not taken to the dealer. the manufacturer was not contacted. the vehicle was not diagnosed or repaired. the failure mileage was 70,000.
Tl* the contact called on behalf of [xxx] who owns a 2012 ford f-150. while the vehicle was reversing out of a parking lot, the steering seized and the service advancetrac warning indicator illuminated. the vehicle was taken to harrison ford (820 n main st, wellington, oh 44090, (888) 684-4989) where it was diagnosed that the power steering gear was faulty and needed to be replaced. vehicle was repaired. the contact referenced nhtsa campaign number: 14v316000(steering), but the vin was not included in the recall. the manufacturer was notified of the failure and did not assist. the failure mileage was 120,000.*pm*jb information redacted pursuant to the freedom of information act (foia), 5 u.s.c. 552(b)(6). *tr
While driving my truck will automatically change gears and the speedometer will go to zero. sometimes it will drop into a low gear, sometime it will jump into and higher gear. on two occasions now my truck has gotten stuck in 5th gear while driving on the highway. when i come to a complete stop it wont downshift below 3rd gear. both times this has happened the speedometer seems to short out as well and goes to zero. if i turn the truck off when this happens the transmission resets and all gears work until the next time it occurs while driving. overall this has now happened 4 times over the past month. i took the truck to the dealership for a transmission related recall, they reprogramed the transmission and the issues has continued to happen. this is a safety hazard and should be address immediately by the ford motor company.
Tl* the contact owns a 2012 ford f-150. while driving approximately 55 mph up a hill, the transmission abruptly downshifted and smoke was present coming from the engine compartment. the vehicle was towed to an independent mechanic who diagnosed that the connect in the transmission line malfunctioned and caused transmission fluid to leak from the vehicle. the transmission lines were repaired, but the vehicle continued to malfunction. while driving up a hill, the transmission abruptly downshifted. while driving down a hill, the transmission would not properly shift. the vehicle was taken to bob maxey ford of howell (2798 east grand river ave, howell, mi) where the lead frame was replaced. the contact was informed that metal shavings were also present inside the transmission, which was not repaired. the contact indicated that the vehicle was previously repaired per service program number: 19n01b to reprogram the transmission. the manufacturer was notified of the failure and provided case number: 24445319. the failure mileage was 96,000.
My transmission sticks in one gear, shifts very hard, and down shifts unexpectedly. there is a recall for this, but ford has not started to fix this problem. it is causing additional damage to my vehicle the more i drive it. the recall was started in febuary of 2019.
Upon startup engine makes a terrible rattle knock noise , upon investigation we came to the conclusion that timing chain stretched and or tensioners and cam phasers may have failed. that is until the oil pressure is high enough to correct this issue
Vehicle shows huge wrench on screen. speedometer/odometer stops working and only have 3rd and 5th gear. next the truck violently downshifted to 1st gear. took to the dealer to have recall fixed. they said they reset the computer and downloaded new software. 2 days later, truck was stuck in 1st gear. pulled over, restarted it and seemed fine. now about once a day, will suddenly shift into 1st at any speed again. seems like they never fixed. back in the shop now so let's see what happens. loss of work, and my safety is a huge concern. paying for a vehicle that is truly unsafe!
Tl* the contact owns a 2012 ford f-150. the contact received a recall notification for nhtsa campaign number: 19v075000 (power train). butler county ford (400 s main st, butler, pa 16001, (724) 287-2766) performed the recall repair. after the repair, the speedometer malfunctioned and several unknown warning indicators illuminated on the instrument panel. the contact returned the vehicle to the dealer the following day. the dealer stated that the manufacturer would not authorize additional repairs to replace the molded lead frame because the vehicle had a restructured title. the contact wondered why the dealer did not inform him of the restructured title during his initial visit. the failure mileage was 88,400.
My 2012 ford f150 has an automatic transmission 6r80. while going 15-20 mph the transmission suddenly and violently downshifts from 2nd to 1st causing the rear tires to slide.this has happened repeatedly. the ford dealership has completed all the known service bulletins which include a software update and a lead frame replacement. this is a dangerous issue on slick roads.
Tl* the contact was speaking on behalf of her son in law who owns 2012 ford f-150. the contact stated that while driving at 40 mph, the vehicle gear downshift without warning to first gear then to neutral. the contact managed to veer to the shoulder of the road, turn the vehicle off and back on in order for the vehicle to resume back to normal. the vehicle was taken to the dealer (david mcdavid ford fort worth 300 w loop 820 s, fort worth, tx 76108 (817) 420-7990) where it was diagnosed that the oss failed and needed to be replaced. the vehicle was not repaired. however, the failure recurred intermittently. the manufacturer was not made aware of the failure. the contact mentioned nhtsa campaign number: 19v075000 (power train) as a possible solution to the failure. the failure mileage was approximately 181,057.
Secondary timing chain link came apart soon after i bought the truck in 2015 at 66,000 miles my warranty paid for it. now it is happening again but on the other side of the engine. 2 time chain links fall apart. ford is not paying for this because my warranty was up 2 months ago and it is not a pre certified truck, miles are 120,066 miles. 2 timing chain links break in 4 years???????? the technicians at santa maria ford said they have never seen anything like this before. ford is denying to pay for this repair!!!!!!!
I was casually driving on the highway just outside of atlanta when all of a sudden i felt a jolt in the rear end and my advanced traction control turned on and stated that there was a problem. i pulled over and called a tow truck to take it to the ford dealership for service. the dealership advised me that somehow my rear axle differential had snapped on the rear driver side. they stated that usually they see that in high lifted trucks or with towing something heavy. my truck is standard lift and i do not tow any trailers and i am thinking it could possibly have been a factory defect as my truck is still pretty young and has only 114000 miles
The truck was in park i had stepped out to check the mail with my children in the truck the next thing i knew my truck was moving like it was in gear i jumped in pressed the break and it still showed it in park i pushed into park harder and it stopped.. i was not on a hill or anything to make it roll and it was actually moving like it was put into gear
After the transmission recall, to prevent the vehicle from downshifting into first gear when at speed, the truck's mileage has dropped drastically. although not a safety issue, it has not been addressed by ford through complaints to it dealerships.
Driving 75mph towing boat. truck shifts into 1st gear. redlined engine, many noises, slipping transmission, step on pedal to accelerate, it goes to 5k before shifting. almost caused major highway disaster. now my family is afraid to ride in vehicle. also when first start up no brakes. recalls where done for both , did not fix either.
Times when i'm driving i used get jerks when shifting gears, i took it to the ford service center they told me that there was recall to adjust the software update. this has been happening since 2012 few months after i had purchased the vehicle. i gave it to the ford service center to update the software. few days after i felt it again but didn't felt it again some time. after a month wiles driving from 4th to 6th gear shift ,i got a big jerk from 4th to 6th gear shift as if i thought the transmission might lock in the middle of the road while i was driving. right after that i took it back at the ford service center and they schedule an appointment for me on monday june17th 2019. i'm writing this because i asked few people i know who owned f150 and they told me the same thing. this could have severe accident wile driving in the middle of the road.
Complaint involves recall 19s07/nhtsa 19v-075. i have had my vehicle in to the ford dealer for service when 1st purchasing and upon receipt of this recall notice. this problem has not been remedied in my vehicle and poses a safety concern. my vehicle still suddenly downshifts or bucks. feels like i am being rear ended. this has happened on multiple occasions, usually at low speeds(05-15mph). transmission does not shift at normal rpm's since recall servicing.
While driving at 60mph the transmission shifted into first without warning, causing a sudden loss of control of the vehicle almost causing multiple accidents as i was in the middle of rush hour traffic. the sudden down shift over spun the motor, resulting in a lower engine knock which a certified ford technician believes is the result of a bearing failure. even though the technician performed a cylinder leakage test in which no indication of valve interference was identified, the ford engineer he was dealing with said he had seen this issue before! this is the same recall issue that has caused multiple accidents! there are dozens of blog posts where this issue has caused, at the very least, multiple trips to the dealerships and in rare cases (not including mine), engine failure. i have calculated the engines rpm's using the formula: axle ratio x vehicle speed x transmission ratio x 336.13) / tire diameter = engine speed. at the time of the incident, the engine's rpm's were at an estimated 9,074 rpm's [(3.55 x 60mph x 4.17 x 336.13)/32.9]. the tachometer only goes to 7,000 rpm's! there are two recalls (19n01 and 19s07) that can cause intermittent shifting into first. the day before the incident, i went to the local ford dealership and was told they couldn't get me in for recall fixes because they needed to "take care of their paying customers first." i have talked with four different ford customer service representatives, the answer has been the same; "there is nothing ford motor company can do for you at this time." i feel ford has been very neglectful in taking responsibility of this issue, not only for myself but the countless other people that have had similar problems.
Tl* the contact owns a 2012 ford f-150. while driving various speeds, the instrument panel indicated that the vehicle downshifted from sixth to first gear and the tires skid. as the contact continued to drive, the vehicle went back into gear; however, the failure recurred. the contact called superior ford (located at 4303 la- 19, zachary, la 70791, (225) 654-2611) and was informed to schedule an appointment. the manufacturer was notified of the failure and informed the contact that the vin was not included in a recall. the vehicle was not diagnosed or repaired. the approximate failure mileage was 30,800.
Tl* the contact owns a 2012 ford f-150. the contact received a recall notice for nhtsa campaign number: 19v075000 (power train). the vehicle was taken to william mizell ford (located at 330 us-25, waynesboro, ga 30830, (706) 554-2114) where the recall repair was performed. as soon as the contact left the dealer, the transmission started to jump when downshifting, and the engine stalled without warning. the vehicle was taken back to the dealer and test driven, but the failure could not be duplicated. the contact stated that he was not aware of the diagnostic testing, but was informed that he would need to drive the vehicle for a while for it to drive normally. the vehicle was not repaired. the manufacturer was not notified of the failure. the approximate failure mileage was 91,000.
The truck often times will not shift out of 1st gear. the rpm will rise and the truck will accelerate quickly then slow back down. when it gets out of 1st gear it will often go back to 1st gear randomly. wrench and check engine lights are frequent. limp mode engages often and the dash will become inactive. fuel economy screen will clear out and all information will be displayed as dash lines. i took the truck to ford to have them look at it under the 16n02 recall and they would not cover it. the coverage only extends to 150,000 miles, but my truck has 163,000. the coverage only covers it if the truck is displaying symptoms and mine did not display anything until this last week.
Tl* the contact owns a 2012 ford f-150. the contact stated that while driving low speeds, the vehicle downshifted to first gear and then decelerated without warning. the failure happened intermittently. dick witham ford located at 2033 la porte rd, waterloo, ia 50702 (319) 234-4200, was made aware of the failure but did not diagnose or repair the vehicle. the manufacturer was contacted. the failure mileage was 61,000.
I had the latest programming recall performed on my 2012 ford f150. since having this performed my truck has gotten worse, when it rains it has down shifted multiple times. it has down shifted from 6th to 2 or 3 when going down a short incline, on level ground or going up a incline at speeds of 45 to 60 mph. the recall made my truck worse also i received a letter today extending the warranty on the output shaft speed sensor to 10 years. if the warranty of a part has been extended to 10 years because of sensor failure problem the sensor may needs to have a recall on it. we have had a rainy year so far, i'm afraid that the problem is going to cause me to have an accident. i'm a senior and dealt with dealerships for many years, i've had automobiles sit in dealerships and never get touched other than they drive around the shop and then state problem could not be duplicated on the paperwork when i pick it up if not for that i would be back to the dealership for a do over. thank you
Jerks at low speeds when shifting if not on the gas. really bad clunk, made our truck hit a pole. keeps doing it and feels like transmission wants to drop out. seems like 1st and second gears.
I purchased my vehicle used from a ford dealership in 2017. i brought it in a few months after i purchased it due to rear tires locking up and a sudden downshift. was told no codes were found wrong and they couldn't duplicate the issue. april 30, 2019 had to have it towed back in. was informed of a recall on lead mane frame for first time. after replacing lead frame vehicle still will not preform properly and i have now been told entire transmission needs to be replaced. i haven't received any letter nor made aware at time of purchase of any recall. ford continues to refuse any remedies to justify this problem. i'm a single mother of 3 and am outraged by the companies failure to notify me of this safety recall. the company refuses to help because my mileage is 152,000 miles. 2,000 over the extended warranty for recall. if i would have been notified in a timely manner of this faulty transmission and possibly deadly recall i would have immediately tended to it. my children are my world and ford cares less about their safety and mine and it is wrong. please help me and my babies.
Vehicle in forward motion at low and highway speeds. transmission shifts unexpectedly! low gear-high gear-4wd-park!!! especially dangerous on highway. need to have repaired to ba able to drive safely.
I have brought my truck in previous times when it was under warranty, i described the same issue every time about a sudden downshift. i have this documented in service sheets i have obtained from them. they said they couldn't replicate the issue or stated it was a reprogramming issue. over time the downshift issue has damaged the transmission and finally led to it completely failing. i was scheduled to have the recall service on tuesday 4-23-2019, on 4-18-19 i was driving on hwy 62 at 35mph the truck did the sudden downshift nearly causing me to be rearended, after the downshift my truck lost all power and it had to be pushed into a parking lot. then i had it towed to the heritage ford dealership. the issue which is finally been addressed as a recall has led to the transmission failure. i have been quoted $4500.00 for the transmission replacement. i would appreciate any help. thank you, information redacted pursuant to the freedom of information act (foia), 5 u.s.c. 552(b)(6). *tr
Truck currently has a recall on it due to it going into first gear unexpectedly. took truck to closest dealership two weeks ago and had the recall service completed. was told to expect my check engine light to be on for three or four days. when it stayed on a week i called them back to discuss the light still being on. rescheduled a follow up appointment for april 12. on april 5 and april 7 the truck had the same problem that the recall was supposed to fix. going approx 50mph when the truck went into first gear with wheel hop and some fish tailing. when this problem occurs the instrument panel will basically go to all zero's and it takes the truck being turned off for the computer to reset and allow truck to drive normally. just wanted to document here since truck goes back to dealership tomorrow.
For over 2 years now an issue with the driver tran module has been causing my truck to go from any gear into first, locking tires up and causing issues while driving at any speed. ** safety recall was made on driver tran module **. however, my truck is now in worse condition without a fix to the issue, but my truck is now shutting the entire computer module down and i have further transmission issues.
Tl* the contact owns a 2012 ford f-150. the contact stated that the transmission warning indicator illuminated. the vehicle was serviced per nhtsa campaign number: 19v075000 (power train) by mainer ford (1724 234th st nw, okarche, ok 73762, (405) 241-9011), but the remedy failed to repair the vehicle. the contact mentioned that the transmission warning indicator remained illuminated after the recall repair. the manufacturer was not made aware of the failure. the failure mileage was unknown. the vin was invalid.
Tl* the contact owns a 2012 ford f-150. while driving various speeds and downshifting gears, the contact heard a loud abnormal noise. there were no warning indicators illuminated. the failure occurred intermittently. five star ford stone mountain (3800 us-78, snellville, ga 30039, (678) 384-4242) was contacted and stated that the failure was typical. the vehicle was not diagnosed or repaired. the vehicle was previously repaired per nhtsa campaign number: 19v075000 (power train), but the failure recurred. the manufacturer was not made aware of the failure. the failure mileage was 59,000.
My vehicle was serviced for recall 19s07 regarding unexpected downshifting in to first gear while driving. after my vehicle was serviced, my truck began downshifting in to first gear while driving. a new recall 19s19 was issued for improper/insufficient repair of original recall 19s07, however my vehicle's vin is not yet showing up under this new recall, but my vehicle is still downshifting in to first gear while driving
The 19s07 recall service was performed on 3/19/19 at bob tomes ford mckinney tx. today, 3/20/19 the downshift occurred on the way to work. i was traveling at approximately 45 mph and it downshifted to 1st gear. i thought this is what the recall was supposed to fix?
On hwy 65mph accelerating speedometer reading went to zero, transmission auto downshift. had to pull over restart and then both the speedometer and transmission worked. after this several starts the speedometer doesn't work and transmission won't shift out of 1st gear
Tl* the contact owns a 2012 ford f-150. the contact took the vehicle to wynn odom ford (4743 washington st, la grange, nc 28551, (888) 452-6761) to be repaired per nhtsa campaign number: 19v075000 (power train). the service manager informed the contact that there were more advanced failures than the recall repair and advised him to call the manufacturer to get further authorization to repair the vehicle. when the contact called the manufacturer on march 25, 2019, he was informed that the recall repair had been completed and they could not assist. the contact was promised to speak with a service manager who said that the recall only authorized the reprogramming and further authorization and a case number were needed to get the repairs completely done. when the vehicle accelerated to shift into a higher gear, it shifted into neutral instead and slowed down to a lower speed. the contact was not assisted or assigned a case number. the vehicle was repaired. the contact was denied being able to speak to a supervisor. the failure mileage was 99,500..*bf consumer received second recall was issued. consumer took the vehicle to recall serviced. vehicle still not shifting properly, in fact it seems to be worse. serviced 5-28-19.*jb
Tl* the contact owns a 2012 ford f-150. while driving various speeds, the vehicle downshifted into first gear without warning. there were no warning indicators illuminated. the vehicle was repaired per nhtsa campaign number: 19v075000 (power train) by brandon ford (9090 e adamo dr, tampa, fl 33619, (813) 246-3673), but the failure recurred. the vehicle was not repaired again. the manufacturer was made aware of the failure. the failure mileage was 130,000.
I received written notice from ford that our 2012 year model, ford, f-150 pickup truck was recalled under ford recall: 19s07 and nhtsa recall: 19v-075. this is the recall having to do with fords pcm programming being unable to cope with an intermittent failure of the transmission oss. the pcm would force the vehicle into 1st gear regardless of current operating speed (mph). i was just informed by a local ford dealership that ford cannot perform a fix on my vehicle because they only repaired the pcm for the v8 engine and have not yet gotten around to the v6 eco boost engines, even though the recall documents i received instruct me to take the truck in for the recall repair. further, the recall documents i received have the vehicle info (vin included) on each of them so one would assume the vin was crosschecked with ford's manufacturing data base to determine if the offered fix actually fit the vehicle. i am told that our vehicle does need the fix but it will not be out until some time later this summer.
I recieved a safety recall notice 19s07/nhtsa recall 19v-075 concerning a recall to reprogram the powertrain control module to prevent unintended downshifit to correct intermittent ailures of the output speed sensor that may result in an unintended downshift to first gear. the proposed correction is inadequate, since it does not address the root cause of the problem, that is, intermittent failure of the output speed sensor. i request that the nhtsa require ford motor company to correct the root cause of the problem, that is, the defect existing in the output speed sensor that may cause unintended downshifts, since failing to correct the root cause of the issue and merely installing a software patch will not result in proper operation of the vehicle, potentially impairing fuel economy and/or proper function and safety during specific driving conditions.
My transmission was shifting out of 6th gear into 1st gear. this was updated by a computer fix that keeps it from shifting to 1st gear but into 5th instead. ford has a customer satisfaction program 16n02 that addresses the same problems i am having and a fix for it. my truck was built at the plant indicated, in the time frame indicated and gives the engine codes as indicated by the bulletin. i have contacted them about it and despite all these findings am being told that it is not their problem to fix. my mileage is below the 150k threshold that is set for this warranty extension on the lead frame problem. their fix to stop it from shifting to 1st gear reduces the chance for a major accident but their refusal to fix the problem in the lead frame still leaves this as a safety issue. a down shift at speed to 5th gear still causes an unindicated and uncontrolled slow down of my truck, the loss of speedometer is also apparently causing some problem with the passenger side airbag when this occurs as i ma getting a message to remove objects from the passenger seat when this occurs. my local ford shop will not try to warranty this problem, i have made 2 different contacts at ford concerning this and they all say it is not one they have marked for having this problem. i can't help that it is not one they thought might be affected, i can only say that it has been and can't get it repaired by them.
Had recall preformed it still is dropping into 1st gear while driving complaint number: 11181584
Tl* the contact owns a 2012 ford f-150. the contact received notification of nhtsa campaign number: 19v075000 (power train). the contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. coggin ford jacksonville (9650 atlantic blvd, jacksonville, fl 32225, (904) 567-3782) was contacted and confirmed that the parts were not available for the recall remedy. the manufacturer was not made aware of the issue. the contact had not experienced a failure. parts distribution disconnect.
2012 ford king ranch 3.5 ecoboost f150 suddenly down shifts during acceleration. it is labored when accelerating, it gets to the 6th gear then suddenly down shifts.this happens when trying to merge on to the highway and also when coming from a complete stop after sitting at a light, then trying to accelerate. it has happened on the highway at 70mph trying to accelerate to 75 mph too. it seems like it loses power.
On the evening of 2/14/19 while returning home from the car wash, my 2012 ford f-150 suddenly downshifted into 1st gear from 6th gear while the vehicle was in motion. there was no loss of control as the road was dry and my speed was 40 mph. the next morning i called the service department at jack demmer ford in wayne mi to speak with my service rep. he did not pick up and did not return my call despite my leaving a detailed message and calling again later in the day. on the morning of 2/19/19 while on the freeway at approximately 70 mph, the vehicle went into what i assume is "limp mode". it dropped from 6th to 5th gear and a wrench logo appeared in the message screen. there was no description of the problem, just a logo. when i got off the freeway, i realized that i no longer had 1st, 2nd, or 6th gear. i arrived at work and called the dealer immediately. they gave me a runaround about how an official fix was not available yet and asked me if i had tried turning the car off and back on again. after insisting repeatedly that something be done, i was told i could bring the truck in the have the power train module "reprogrammed". this was done at no cost but i was told this was all they could do for me at this time. the next day in the evening, the truck downshifted to second gear while i was on the freeway doing 55 miles an hour. the road was wet and starting to freeze. i nearly lost control of the vehicle but was able to regain control. the truck returned to normal after this. as of 2/20/19, no further contact has been made with ford regarding the issue. i will be calling tomorrow to customer care about this issue and demanding a rental car until the vehicle is safe to drive. it is my understanding that this is related to recall 19s07. the transmission output speed sensor is reported to be losing communication with the powertrain control module, causing a downshift.
Tl* the contact owns a 2012 ford f-150. while driving approximately 55 mph, the vehicle suddenly downshifted. the contact pulled the vehicle over to the side of the road and then drove to hardy family ford (1255 marietta hwy, dallas, ga 30157) where it was diagnosed that the transmission needed to be replaced. the contact received a recall notification for nhtsa campaign number: 19v075000 (power train). the vehicle was taken to an independent mechanic and repaired. the manufacturer was notified of the failure. the approximate failure mileage was 150,000.
