Headlights constantly go out. research online proves prius vehicles with hid bulb systems have ballast issues causing constant blowing of either bulbs or ballast failure. not only is safety constantly compromised, but it is illegal when a headlight goes out driving down the road.
The headlight lenses covers repeatedly clouds over to the point i can hardly see the roadway at night. dealer have cleanse the lenses how has last for about a month. the dealer now says these need to be replace at a cost to me for $750. i believe this is manufacture's defect and it is a serious safety issue for a recall. the dealer should develop a fix and replace these lenses without charging the vehicle owner.
Headlight bulbs fail frequently, several times per year. i called toyota in january 2020. i was informed they had a brief warranty extension to cover the issue, but that has ended. they've refused to offer any assistance. they claim they notified me of the issue, but they clearly did not; the address they claimed to have sent it to was not my address. this is a safety issue, because the bulbs fail so frequently and are not owner-replaceable. i hope you can help. thank you.

The headlights on my 2005 prius continue to go out. we replace the bulbs but even with brand new bulbs will be driving and after about 60 seconds of headlights being on the light will turn off. sometimes only one turns off but often bolt turn off and we're in complete darkness driving on the road. we have to toggle the headlight control off and on several times before they turn on again, which leaves us driving in complete darkness and it's a big safety hazard! again, brand new bulbs have this issue, too. getting the whole headlight and computer behind it replaced is very expensive. this is a big safety issue and it has been happening and getting worse for over a year.
For a few months, the left hand side front headlight has gone out, if i turn the lights off and then back on again, it comes back on. however last night, while driving on a dark nc highway both lights went out. they came back on when i turned the lights on an off again. this happened two more times. it is clearly a safety issue that i see many other prius owners experience. it is entirely possible both lights have been going out for some time, since driving on dark country highways at 70 mph is not something i regularly do.
Headlights go out after a few days.. replaced over 8 bulbs. checked connectors, checked wiring.. no problem found and the problem still exist.
Headlights intermittently fail. sometimes one headlight will go out and other times both. so far, if i detect one or both headlights are out, turning the headlight switch off and back on will correct the problem. depending on street lighting, i am not always able to detect if the headlights are off until another car flashes to let me know or i enter a dark roadway where it becomes obvious.
The driver side headlight, running high beams and low beams, have stopped working despite changing the bulbs. it seems to be an issue with the ballast going out and/or just not working anymore. this seems like a major safety issue.
The passenger side front headlight started going off automatically after a while of turning on. if i turn it off and on again, it lights up and then first dims and then turns off on its own. the lights have been replaced and it did not solve the problem. this is very dangerous for night time driving as the drivers will never know when the headlights will go out. this is not a random incident; it is one of the most common problems of gen 2 prius (04-09) that i found online. i am attaching a document that lists some online forums where other prius owners have reported similar problems. toyota settled a lawsuit for 06-09 prius, yet there is no recall for the headlight issue of gen 2 prius.
