I am entering a safety complaint regarding the headlights on my toyota 2013 rav4. today, i went to my local car repairman to fix a headlight that blew out. this is the 3rd time in 6 months. in september, we had to completely change both headlights because parts in the headlights were melting. yet, less than 6 months later, my passenger headlight blew out and needed to be replaced. there is something systematically wrong with my headlights. according to my car mechanic, it looks like they are going to continue to burn out. this creates hazardous driving conditions for me and my passengers.

The gas tank will not fill to the full capacity and gauge is inaccurate. the fuel tank size is 14 gallons but i discovered after owning the car for a month that the tank will not fill completely in fact it only fills to 10 gallons meaning the stated vehicle range is false advertising. i have contacted toyota and opened a case but receive no update or feedback.
The fuel tank is supposed to take 14.5 gallons but in reality only takes 9.5 gallons, called dealer and they state they have no knowledge of problem even though there are several articles regarding issue.
I was alone around 630pm driving straight east on i10 twin spans in the left lane going 70ish mph when i heard a loud pop sound. overhead i could hear a little whistling and rattling coming from the sunroof shade that was closed. i quickly got into the right lane and slowed to about 50 mph. i immediately stopped at the toyota dealership for them to review the vehicle. they claim a rock hit the glass, however, there was no one in front of me to throw up a rock. it was not raining. it was slightly windy. this occurred on tuesday, december 17, 2019.
