The driver's safety belt plate did not latch into the buckle. *jb

Consumer states while traveling at 40 mph, traffic ahead was coming to a stop. consumer applied the brake, but the vehicle did not stop. consumer states accelerator pedal would not release, it stuck to the floor. vehicle accelerated at a high speed before rear ending another vehicle, after consumer was forced to use emergency brake.*jb during the collision the vehicle spin and went out of control, the air bags and seat belt failed and the consumer was also unable to get the vehicle out of drive. *yh

When fastening child passenger seats into left and right rear positions of vehicle, restraint belt buckles broke and came apart. dealership was examining vehicle ,and would perform any repairs. please provide any additional information/attachments. *ak
